Online Refills

New Prescription:

It will be necessary for you to fax, mail, or bring in your prescription.  It is also acceptable for your doctor to call in a prescription. 

Transfer Prescription:

It will be necessary to provide us with the name and phone number of the pharmacy that has filled it for you in the past and your doctor’s name and phone number. 

Refill Prescription:

Simply let us know you would like a refill of a prescription currently on file or complete the form below. You may select whether you would like to pick up your prescription at our Clearwater or Seminole location. If you prefer we can ship or deliver your prescription to your home or office. We offer FREE delivery within a 10 mile radius.

Vacation Prescription Refills:

We can ship to you anywhere in the USA when you are on vacation or while you are away for the summer. Please allow time for compounding and shipping.
